TRANSPORTATION TO CARE

Our MISSION is to provide reliable transportation to clients with barriers to accessing in-person care in compliance with HHSC and Joint Commission standards. 

Transportation may be provided through:

(1) company-owned vehicles

(2) contracted ride services (e.g., Lyft Concierge)

(3) DART GoPass® Tap transit cards

TRANSPORTATION CONSENT FORM

CELL PHONE ACCESS TO CARE

We want to ensure clients without reliable communication have access to phones for care coordination while maintaining compliance with HIPAA, 42 CFR Part 2, and Joint Commission standards. 

Phones provided are the property of Life Point Healthcare Services of Texas (LPH of Texas).

Devices may only be used for therapy coordination, telehealth sessions, and crisis/emergency communication.

Clients must sign the Phone Issuance & Use Agreement and SMS/Texting Consent form.

PROGRAM POLICIES

  • All communications must comply with HIPAA and 42 CFR Part 2. - Clients must sign: - Phone Issuance & Use Agreement - SMS/Texting Consent - Telehealth Consent - Transportation Consent - 42 CFR Part 2 Authorization (if applicable) - Clients can revoke consent at any time. 

  • All phones and transit cards are tracked by unique ID numbers. - Inventory must reconcile monthly. - Lost or stolen devices are remotely wiped within 24 hours. 

  • Phones and rides are provided solely to reduce barriers to treatment, not as marketing incentives. - Eligibility is based on clinical need and documented using the Eligibility Screen Form
