Ensure clients without reliable communication have access to phones for care coordination while maintaining compliance with HIPAA, 42 CFR Part 2, and Joint Commission standards. 

Phones provided are property of Life Point Healthcare Services of Texas (LPH of Texas). - Devices may only be used for therapy coordination, telehealth sessions, and crisis/emergency communication. - Clients must sign the Phone Issuance & Use Agreement and SMS/Texting Consent form. – (Pending: All phones will be secured with passcodes and managed through a Mobile Device Management (MDM) system with remote-wipe capability.) - Staff must document any clinically relevant communication in the EMR. - PHI is only shared via HIPAA-compliant platforms (secure telehealth/video/text). SMS use requires signed risk acknowledgement.
